Forehead and glabella filler aims to soften deep static lines that formed through years of expression and are now visible even when the face is at rest. This is one of the most vascularly demanding regions of the face, which makes candidate selection, correct depth and the principle of small volumes especially important here. It is usually considered as part of a plan alongside botox rather than on its own.
Static lines versus dynamic lines
A dynamic line appears only when you frown or raise your brows, and its source is muscle movement. A static line is etched into the skin and visible even at complete rest. Botox works on dynamic lines and can slow the deepening of static ones over time. Where a line is already well established in the skin, however, botox alone may not be enough. That is precisely where filler has a role.
Which areas are addressed
- Horizontal forehead lines
- Vertical frown lines between the brows
- The horizontal line at the root of the nose
- Limited support of a flattened forehead surface when needed
Who it suits, and who needs closer assessment
It is considered for people with deep lines that persist despite botox, or with prominent static lines. It is not chosen for every patient in this region; the doctor weighs the depth of the line, skin thickness and vascular anatomy together and may recommend a different approach where filler is not appropriate. It is not performed during pregnancy and breastfeeding, with an active infection in the area, in bleeding disorders or uncontrolled autoimmune disease. Suitability must be determined by a medical examination.
How the session proceeds
The lines are examined at rest and in movement, and it is decided whether botox should come first or whether the two should be combined. Often botox is planned first and the remaining line is reassessed once its effect has settled. At the application stage the skin is cleaned, points and depth are marked carefully and a topical anaesthetic can be applied for comfort. A small amount of filler is placed at the appropriate depth, slowly and under control. Skin colour and what the patient feels are monitored throughout. It is completed in an outpatient setting.
Onset and duration of the effect
The softening begins to show after the application and becomes clearer once the mild swelling of the first days settles. Duration depends on the product, the amount and the individual and is usually described in months. When planned together with botox the two treatments last different lengths of time, and the review schedule reflects that. Results vary from person to person.
Aftercare
- Avoid rubbing or pressing the area on the first day.
- Leave hats, bandanas and tight headbands aside for a few days.
- Stay away from saunas, steam rooms and intense exercise in the first days.
- Small bruises or tenderness can occur and are usually temporary.
- If you notice blanching, increasing pain or any change in vision, contact the clinic without delay.
Points that are often confused
The purpose of filler is not to stop expression; that is what botox does. Filler only supports the gap within a settled line. When this distinction is missed, expectations are set wrongly. The second common misconception is that forehead lines will disappear entirely. The aim here is to soften the shadow, not to erase the line. The third concerns quantity: the forehead and glabella do not forgive excess product, and a measured approach is preferred for both appearance and safety.
Factors that influence the result
- How old the line is: in a line that has been etched in for years, the softening that can be expected is more limited.
- Skin thickness: product shows more readily in thin skin, so the amount is kept even more conservative.
- Expression habits: in people who frown frequently, the result can regress quickly without botox support.
- Sun and smoking: both affect skin quality and therefore the long term appearance.
- Sleep position: always sleeping on the same side can influence how forehead lines develop.
These are weighed together at the visit. Results vary from person to person.
Useful things to know before the visit
Please tell us about the medications you take, especially blood thinning drugs and supplements, any known allergies and every treatment this area has had before. Knowing when your last botox session was directly affects the plan. If you have an important event coming up, arranging the treatment at least a week beforehand makes for a more comfortable process.
